Important Information About the AvMed Online Provider Directory

AvMed Health Plan offers two (2) Medicare Advantage PPO Plans in South Florida, one in Miami-Dade county and one in Broward county. The plans are called AvMed Medicare Preferred PPO.

AvMed Medicare Preferred PPO offers a full network of AvMed participating Primary Care Physicians, Specialists, Hospitals and other providers. As a member of AvMed Medicare Preferred PPO you can use any doctor who is part of the AvMed Medicare Preferred PPO network or providers who accept Medicare but do not participate in AvMed's Medicare PPO network.

AvMed Medicare Preferred PPO members will not have to select a primary care physician and will not need a referral to visit specialists. Physicians who accept Medicare but do not participate in AvMed's Medicare Preferred PPO can still treat AvMed Medicare Preferred PPO members at a nonparticipating provider rate. AvMed Medicare will pay for all covered benefits both in and out of network. For more information, please call the number at the end of this introduction.

Important Information for AvMed Medicare Preferred Members

Some Medical Providers have chosen not to be Medicare Participating providers at regular Medicare rates. These providers are called "Opt Out" providers and can't bill Medicare or a Medicare Advantage Plan for any service or supplies given to you and all his/her other Medicare patients for at least 2 years. You can enter into a private contract with an "Opt Out" provider if you wish, however, there are no limits on what you can be charged for services under a private contract. You must pay the full amount of the bill unless the services are for emergent or urgent care.

"Opt Out" providers are not participating Medicare providers with AvMed Health Plans. However, because AvMed Medicare Preferred Members may utilize non-participating providers for certain services, there is a risk that you will be financially responsible for services rendered by "Opt Out" providers.

Before seeing a non-participating provider for services other than emergency or urgent care, it is important to make sure that provider has not "Opted Out" of Medicare. If the provider has "Opted Out", you will be responsible for 100% of charges. AvMed and regular Medicare will not make payment to an "Opt Out" provider unless it is urgent or emergency care.
